Prior to your FIRST appointment please complete your new patient intake forms:
- The completed forms allow the acupuncturist to thoroughly assess and give you a personalized treatment
- The intake forms can be completed:
- On line (which do not need to be printed)
- At the clinic. (If completed at the clinic, please arrive 30 minutes earlier than your scheduled treatment time, to allow sufficient time to complete the paperwork, and to receive a full treatment)
How to prepare for your first visit:
- The initial appointment will last about 90 minutes. The acupuncturist will do a thorough intake and assessment, and then a full acupuncture treatment.
- Wear loose/comfortable clothing, with shirt sleeves and pant legs that can be pushed/rolled up.
- For your comfort, you may want to wear layers, until you get used to the office temperature
- Don't come on an empty stomach - most people do better with little bit of food in their stomachs before their treatments.
- Our clinic is a "Scent-free" zone
- Before entering clinic, turn off cell phones or put to low vibrate
- Please cancel with a minimum 24-hour notice to prevent fees and allow us to schedule other patients that need an appointment
- Arrive on time (later than 15 minutes may result in a shortened treatment or rescheduling)
